What does a medical invoice need to include?

In order for us to process your claim, you need to send us an itemized medical bill – otherwise known as a “super bill” or “medical invoice.” The itemized bill must include the following information:

1. Patient’s name

2. Provider’s name and National Provider Identifier (NPI) number

3. Date(s) of service

4. Amount charged by the provider for each service

5. CPT codes for the services

6. Reason for the services (including all ICD-10 diagnosis codes)

Note: in the case of prescription drugs, Sidecar Health requires the drug name, form, strength, and dosage or the National Drug Code (NDC).
